Surgeon pathway approvals

A clinical decision point taken from workflow sketch to shipped, auditable system — over SMS rather than an app login, because that is how surgeons actually work.

Outcome

An approval record that survives duplicate submissions and retries — and stands up in regulatory review.

01 · Context

BIBO is a TGA-registered Software as a Medical Device for post-operative rehabilitation. Surgeons had to approve each patient's rehab pathway, but would not log into another dashboard to do it — and every approval still had to be auditable.

02 · Approach

Scoped the decision points with the CEO and clinical lead, then chose SMS delivery over an app login. Built the capture path end to end: Twilio delivery, an idempotent approval endpoint, and a data model designed around duplicate submissions and retries rather than patched for them.

03 · Outcome

Approvals reconcile against patient records and produce a record fit for regulatory review. Deliverables were scoped against the existing system's debt, so trade-offs were explicit rather than discovered late.

The Postgres ↔ Firestore boundary

One question, answered once: which store owns which truth. Clinical records in Postgres, real-time patient state in Firestore — and a read bill that stopped growing.

01 · Context

A Next.js clinician dashboard and a Flutter patient app both read and wrote across two stores with no agreed boundary. Records of truth blurred, and Firestore reads climbed with every new poller.

02 · Approach

Drew the boundary explicitly — clinical records of truth in Postgres, real-time patient state in Firestore — then fixed poller scan patterns and denormalised the hot paths behind it. Established Alembic migration history on a live production database with no prior versioning and reconciled drifted dev and prod schemas.

03 · Outcome

Firestore read costs down 25%, schema change reviewable and reversible, and a rule new features can be checked against instead of a habit each developer had to guess at.

Isolating an exposed surface

The clinical decision system as a standalone service on its own domain — externally reachable, but unable to take the main dashboard down with it.

Outcome

Three security findings remediated, and a staging environment releases could be proven against before reaching patients.

01 · Context

Approvals had to be reachable from outside the platform, and the platform itself carried committed credentials, plaintext secrets and unauthenticated endpoints.

02 · Approach

Ran the decision system as its own Next.js service on a separate domain with an idempotent write path, so the exposed surface stayed structurally separate while approvals remained reconcilable with patient records. Chose polling over WebSockets for approval-state propagation once connection-state bugs proved costlier than the latency they saved.

03 · Outcome

Secrets moved to Secret Manager, Cloud Run hardened against bot traffic, and an isolated staging environment stood up on Cloudflare DNS with a GCP load balancer, NEGs and Cloud Armor.

04 · Creo IT · 2023–25

Urbanaut: native → Flutter

Led a migration off native code onto one Flutter codebase, cutting the cost of every feature that came after it — alongside screening apps used by 300,000+ people.

01 · Context

Creo IT shipped medical screening software — iBreastExam and CervAIcal, deployed across countries to more than 300,000 users — alongside Urbanaut, which was carrying separate native codebases.

02 · Approach

Led the migration to Flutter and held a cross-functional role optimising the existing products and migrating frameworks — shipping features inside the constraints of regulated clinical software throughout.

03 · Outcome

One codebase instead of two: development resources and long-term maintenance both came down, and feature parity stopped being a per-platform negotiation.

About

I like problems where being wrong is expensive.

Most of my work has been inside regulated clinical software, where a shortcut shows up later as a finding. That has made me careful in a specific way: draw the boundary before the feature, make schema change reversible, pick the boring mechanism when the clever one adds failure modes.

I work across the stack — backend, dashboard, mobile, infrastructure — and I like being in the room where the product decision gets made, not just the ticket that follows it. Currently taking on contract engineering work and landing pages for businesses.
